IMPORTANT: Please use the "New Team Registration Form (Excel)" save it to your computer, fill it out, then email it to DMRegister@gmail.com. You must have Excel installed on your computer for this form to work.

• Click on New Team Registration Form (Excel\MSWorks)
• A Window will popup, click on "Save" NOT OPEN
• Choose a location to save the document on your computer
• Go to the location you saved it, and open it
• When you have finished filling it in, save and email it to Doreen Meldrum as an attachment, DO NOT copy and paste into an email message as the format will be lost.

If you don't have Excel or are unable to fill out the form, please use the "New Team Registration (Adobe PDF)" which will allow you to print a copy of the form and fill it out manually then give the form to Doreen Meldrum in person.
